How much does a new bathroom cost?

There is no real answer to this. There are many factors involved. The bespoke nature of the design will play a part. Also if you use the higher end goods/brands, then the costs will obviously be higher. In my experience you do, to some extent, get what you pay for, but having said that a beautiful bathroom can be achieved with even a moderate budget.

What is the best design layout for a small bathroom?

I think you need to take it on a job by job basis according to the customers wants/needs. I'd be more inclined to use slimline furniture to help make the most of the space available and most feeds and wastes can be moved around to help implement this. I'd also stick to a lighter colour scheme to help create a feeling of spaciousness.
